Enrollment Agreement

This agreement constitutes a binding contract between the student and the school when signed by the applicable parties and accepted by the school. By signing, the student certifies that the complete agreement has been read. The student will receive an exact copy of the signed contract and should retain it to protect their rights.

The school reserves the right to change start dates based on enrollment, staff availability, and other considerations.

Catalog Acknowledgement

I acknowledge that I have received, read, and understand a copy of the school catalog. My signature certifies that I have read, understand, and agree to comply with its contents. The institution’s cancellation and refund policies have been clearly explained to me, and I will receive a copy of the fully executed agreement and catalog.

General Terms of Agreement

The institution shall provide programs of study that meet minimum curriculum requirements prescribed by the state regulatory agency.

The institution may change kit contents, textbooks, dress code, curriculum format, teaching materials, or educational methods at its discretion.

The institution will grant a diploma of graduation and an Official Transcript of Hours for the applicable course when the student has successfully completed all phases of study, required tests, practical assignments, final comprehensive written and practical examinations, state program requirements, exit paperwork and an exit interview, and has made satisfactory arrangements for all debts owed to the school.

The institution will issue an Official Transcript of Hours to a student who withdraws before program completion after the student completes the required exit paperwork, attends an exit interview, and pays all debts owed to the school.

The institution will assist each graduate with job placement after successful program completion; however, the school does not guarantee employment.

The institution may terminate enrollment for noncompliance with school policies, this agreement, or state laws and regulations; improper conduct; conduct that causes or could cause bodily harm to a client, student, or school employee; willful destruction of school property; theft; or any illegal act.

Possession, use, or sale of drugs or alcoholic substances is prohibited during school time and on school premises. A violation is a serious offense that may result in immediate termination and notification of law enforcement.

The student agrees to pay applicable school and state fees and provide all required registration paperwork in a timely manner.

The student agrees to comply with standards of conduct, school policies, state laws and regulations, and educational requirements, including clinic assignments.

The student agrees not to refuse client services or other program requirements.

The student agrees to provide all required financial-aid documents, if applicable, within the designated time frame.

The student agrees to comply with the school dress code and maintain a professional image appropriate to the program.

The student agrees to follow the assigned program schedule, which may change at the school’s discretion.

The student agrees to attend scheduled theory classes for the duration of the course of study, even when required tests have already been taken and passed.

Terms of Payment

Students may pay educational costs by cash, check, money order, or credit card. Tuition is due in full on or before the first day of class unless the student has an approved payment plan.

A student unable to pay tuition and fees in full may arrange a weekly, biweekly, or monthly school payment plan. Contracts are not sold to third parties; however, the school may assign unpaid balances to an outside collection agency.

For the Preserva payment plan, the $1,000 deposit is included in tuition and contains a $150 nonrefundable registration fee. Equal installments are rounded up to the nearest whole dollar, so the scheduled total may be slightly higher than the remaining balance.

Affirm and Klarna financing are subject to the financing provider’s approval and terms. Enrollment is not approved until Stripe confirms successful payment, unless the school records an authorized manual approval.

Cancellation and Refund Policy

This policy applies to students who voluntarily withdraw or are terminated from a program. The official termination date is the last date of attendance. A student may cancel the signed enrollment agreement in writing within three business days, whether or not training has begun.

All monies will be refunded if the application is not accepted. If the school does not accept the applicant, or the student cancels within three business days after signing the agreement and making the initial payment, all monies will be refunded.

Cancellation after the third business day but before the first class will result in a refund of all monies paid except the $150 registration fee and the cost of books, materials, and supply kits that were preordered.

Refunds are not granted for issued or accepted books, materials, supply kits, lab fees, or educational materials. A $150 termination fee will be assessed.

Students are responsible for all tuition, fees, books, and supplies for the period of financial obligation they are attending, plus any prior account balance. A withdrawn student who re-enters will be charged a $100 re-entry fee.

Official cancellation must be made in person or by certified mail. The cancellation date is the postmark on written notification or the date the notice is delivered to the school in person.

After attendance begins and through 40% completion of the program, the refund is calculated pro rata based on scheduled hours completed compared with total program hours. From 40.01% through 49.9% completion, the student owes 70% of tuition. At 50% completion or more, the student owes 100% of tuition and no tuition refund is due.

The termination date for refund calculations is the last date of actual attendance unless written notice is received. Refunds will be made within 30 days of the date of determination or receipt of the cancellation notice. The date of determination will be within 14 days after the last date of attendance.

If the institution cancels a class or program before attendance begins, the student will receive a 100% refund. If the school permanently closes and no longer offers instruction after enrollment, the student is entitled to a 100% refund.

If a student on an approved leave of absence states that they will not return, the withdrawal determination date is the earlier of the scheduled return date or the date the student notifies the institution.

The student’s rights under this agreement may not be assigned to another person.
